The National Transportation Safety Board (NTSB) investigates civil aviation accidents. When notified of any major accident in the US, it deploys a "Go Team" to investigate the cause of the accident. Each on-call investigator on a Go Team has a specialty and is responsible for a unique aspect of the investigation. In aviation investigations, these investigators may include:

  1. Operations Investigator (documents history of pilot and crewmember duties in the days leading up to the incident)
  1. Human Performance Investigator (documents crew performance before the incident and evaluates factors that may be attributed to human error, including alcohol, medications, and fatigue)
  1. Structures Investigators (exams the wreckage and scene of the accident and calculates estimated impact angles to determine how the plane was travelling prior to the accident)
  1. System Investigator (studies the components of the aircraft's electrical, hydraulic, pneumatic and associated systems, and flight control system instruments and elements)
  1. Weather Investigator (gathers relevant weather data from national weather service, meteorologists and local TV stations)
  1. Survival Factors Investigator (documents impact forces and injuries, community emergency planning, response of fire and rescue teams, and evacuation)
  1. Air Traffic Control Investigator (collects transcripts of controller-pilot radio transmissions and acquires air traffic control radar data)
  1. Powerplants Investigator (exams engines, propellers, and engine accessories)

After investigating an incident, the NTSB will make suggestions to the Federal Aviation Administration (FAA) to reduce the likelihood of future incidents and improve aviation safety. Only about 82% of more than 12,000 recommendations have been adopted. When companies fail to fix defects in their aircraft, it jeopardizes the safety of pilots, passengers, and people on the ground.
